Payment and Cancellation Policy

Payment is due at time of service. We accept all major credit cards, HSA, and Flex Spending cards. A $50 deposit is required for first time patients. Credit will be applid to payment total. We do not bill insurance, but we can provide a superbill that you may submit to your insurance company. If you are experiencing financial hardship please let us know and we can set up a payment plan. At least 50% of payment total is due at the first appointment and monthly payments for the remaining balance can be scheduled.

 

Cancellation Policy Summary

In order to be respectful of the medical needs of other patients, please call us promptly if you are unable to show up for an appointment. If sufficient notice of cancellation is given – 24 hours or more – no charges will apply. If you do not reach the receptionist, you may leave a detailed message on our voicemail or text option. We will return your call and give you the next available appointment time.

A late cancellation is when a patient fails to cancel their scheduled appointment at least 24-hours prior to the appointment. Late cancellations will be subject to a “Cancellation” fee of $25.00. In the event of an actual emergency where prior notice could not be given, a one-time exception may be granted.


No-Show Policy

A “no-show” is someone who misses an appointment without cancelling, or rescheduling, at least 24 hours prior. A failure to be present at the time of a scheduled appointment will be recorded in your medical record as a “no-show.” Patients who fail to show for their scheduled appointment shall be subject to a “No Show” fee of $25.00 for a return visit or $50 for a new patient visit. In the event of an actual emergency where prior notice could not be given, a one-time exception may be granted.


Package Agreement:

Packages are non-refundable and can not be used with other discounts or specials. After the expiration date, the remaining credit will be good for service price only. Discount will be void. Balance may be transferred to a recipient of your choice or used towards other services or products.
