Payment and Cancellation Policy

A non-refundable payment is due in full at the time of service. We do not bill your insurance company. Upon request, we will give you a service summary with codes that you may submit to your insurance company. Many patients receive reimbursement when they submit this receipt. We accept checks and major credit cards; a $10.00 service fee will be charged for all returned ​checks. If you are experiencing financial hardship please let us know when you schedule your appointment. We will work with you to create a payment plan. All patients are required to pay at least 50% of the charges on the day of service.

Cancellation Policy Summary

If sufficient notice of cancellation is given – 24 hours or more – no charges will apply. Less than 24-hour notice or no-shows will be charged at 50% of the scheduled visit. This applies to first-time visits as well.

When you schedule an appointment at Cameron Wellness we set aside enough time to provide you with the highest quality care. Should you need to cancel, or reschedule, an appointment please contact our office as soon as possible and no later than 24 hours prior to your scheduled appointment. This gives us time to schedule other patients who may be waiting for an appointment. Appointment Cancellation/No Show Policy details are listed below:

Cancellation of an appointment:

​In order to be respectful of the medical needs of other patients, please be courteous and call Cameron Wellness Center promptly if you are unable to show up for an appointment. This time will be reallocated to someone who is in need of treatment. If it is necessary to cancel your scheduled appointment, we require that you call at least 24 hours in advance. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care.

How to cancel your appointment:

​To cancel appointments, please call 801-486-4226. If you do not reach the receptionist, you may leave a detailed message on our voicemail. If you would like to reschedule your appointment, please leave your phone number. We will return your call and give you the next available appointment time.

Late Cancellations:

​ A late cancellation is considered when a patient fails to cancel their scheduled appointment within 24-hours prior to the appointment. Patients who fail to notify the office within 24 hours of their scheduled appointment time will be subject to a “Cancellation” fee of $25.00. In the event of an actual emergency where prior notice could not be given, consideration will be given, and a one-time exception may be granted.

No-Show Policy:

​ A “no-show” is someone who misses an appointment without canceling or rescheduling in an adequate manner. A failure to be present at the time of a scheduled appointment will be recorded in your medical record as a “no-show.” Patients who fail to show for their scheduled appointment shall be subject to a “No Show” fee of $25.00. In the event of an actual emergency where prior notice could not be given, consideration will be given, and a one-time exception may be granted.
